UW Medicine researchers were awarded a $12.7 million grant from the Patient-Centered Outcomes Research Institute to improve post-acute care for patients with a traumatic brain injury (TBI). The principal investigators of the research team — Jeanne Hoffman, Ph.D., UW professor in the Department of Rehabilitation Medicine, and Jesse Fann, M.D., MPH, UW professor in the Department of Psychiatry and Behavioral Sciences — will enroll 900 people with moderate to severe TBI in their study. All of the patients will receive standardized discharge care, but half of them will also work with a care manager who will assess for unmet needs and assist with coordination of care via telephone for six months. The research team will then determine whether this coordinated-care approach improves outcomes for TBI patients.
