Your Letter
Please submit a separate letter for each scholarship or student support fund you’re receiving this year. (Note: not all scholarships and support funds require a thank-you letter. You can skip this section if a letter is not required.)
Share Your Photo(s)
Donors love to see photos of the students they’re helping! You can share formal or informal pictures, at work or having fun, by yourself or with your family. We’ll resize your photo(s) and put them at the top of your letter.
For privacy reasons, please do not include photos with patients. We won’t be able to use them.
The maximum file size is 5 MB. To submit a photo with a larger file size, please email it to Tricia Riley at pjriley@uw.edu.
How We Use Your Information
Your thank-you letter and pictures will be shared with your scholarship donors. Sometimes, we also use student stories and pictures in other communications, like newsletters or emails. We will always contact you and ask for your permission before using your letter or photo. Please check the box below to show that you have read and understood this section.
