We can help you create a custom tribute webpage with pictures and personal memories about the person you’d like to honor. And it’s easy to share your page with friends, family and community members who would like to make a gift or leave a personal note.

Frequently Asked Questions

Just fill out a short form, and our staff will create the page for the person you’d like to honor. You will receive an email once the page is complete, giving you access to the page and your own URL to share with friends and loved ones. See a sample tribute page.

Please include the name of the person you’re paying tribute to, and, if you know it, the name of the fund being used to honor them.

We suggest that you use text similar to this:

The family requests that memorial gifts be made in support of the [program/hospital/research area] at UW Medicine. Checks should be made payable to the “UW Foundation” and mailed to: UW Medicine Advancement, Attn.: Gift Processing, Box 358045, Seattle, WA 98195‐8045. Please indicate that the gift is in memory of [name]. Or you may make a gift online. (Note: If you choose to create a tribute page, include the URL for that page here.)

If you give us the name and address of the next-of-kin (or the name of the person you’re honoring), we will be sure to let them know about your thoughtful gesture.
